We’re building a top-tier home health team and looking for skilled RNs to join us in Admissions and Case Management roles in either Home Health OR Hospice. Whether you’re ready to take on a full caseload or looking for flexible PRN hours, your expertise is valued here. $95,000–$120,000/year for full-time, based on experience Top pay for RNs with OASIS and/or hospice field experience Performance bonuses — earn more based on your productivity Competitive per-visit rates for part-time and PRN staff Mileage reimbursement, PTO, health insurance, and more (for eligible roles) What You’ll Do: Home Health: Complete Start of Care, Recertification, and Discharge OASIS assessments (home health) Manage a home health caseload with clinical autonomy Collaborate closely with our supportive, nurse-led leadership team Hospice: Conduct initial and ongoing nursing assessments to determine care needs Manage symptom control Provide compassionate end-of-life care and support for hospice patients and families

Requirements

Active RN license in Colorado
Experience in Home Health OR Hospice
OASIS knowledge required for Home Health

Nice To Haves

Management experience highly valued
Medicare home health experience is highly preferred

Responsibilities
