Home Health Registered Nurse
About The Position
Our organization is seeking a skilled and compassionate Registered Nurse (RN) to join our growing team as a 1099 independent contractor working full-time hours to support clients primarily throughout Philadelphia and Montgomery County . This role is ideal for a flexible, patient-focused nurse who is comfortable working with a diverse range of patients in the community and assisting with the intake and onboarding of new referrals. The RN will play a key role in delivering high-quality clinical care while supporting our office with new patient admissions and ongoing case management.
Requirements
- Minimum of one (1) year of clinical or home health experience preferred
- Strong assessment, critical thinking, and clinical judgment skills
- Knowledge of infection control protocols, nursing best practices, and safety standards
- Excellent communication and interpersonal skills with patients, families, and care teams
- Accurate and timely documentation using electronic health record systems
Nice To Haves
- Experience with Axxess and/or HHAeXchange is a plus
- A dependable and compassionate RN who thrives in a community-based setting, values flexibility, and is comfortable working as a 1099 contractor with consistent full-time hours , while making a meaningful impact on patients’ lives.
Responsibilities
- Provide safe, competent care to tracheostomy and ventilator-dependent patients
- Support bedbound and medically complex patients in the home setting
- Perform skilled wound care in accordance with physician orders and best practices
- Provide shift-based nursing care and conduct skilled visits as needed
- Open and manage cases to help streamline new patient referral onboarding and intake
- Maintain a flexible schedule while committing to full-time hours
- Willing and trainable to learn and complete OASIS Start of Care (SOC) admissions
