Requirements
- Active RN license in Michigan
- 2+ years of clinical nursing experience (Home Health preferred)
- Epic or Home Care EMR Experience
- Strong communication, assessment, and organizational skills
- Compassionate, dependable, and mission-minded
- Valid driver's license and reliable transportation required

Responsibilities
- Provide skilled nursing care, assessments, and interventions in the home
- Serve as case manager and patient advocate
- Collaborate with interdisciplinary care teams
