Job Summary: We are seeking a skilled and compassionate Home Health Registered Nurse (RN) to join our team. In this role, you will be responsible for delivering high-quality nursing care to patients in their homes, coordinating care with the interdisciplinary team, and educating patients and their families on disease management and health maintenance. As a Home Health RN, you will provide skilled nursing care to patients according to physician orders and individualized care plans. You will conduct patient assessments, monitor vital signs, and evaluate the effectiveness of care plans. Your role will also involve administering medications, wound care, IV therapy, and other nursing interventions as required. You will educate patients and caregivers on disease management, medication administration, and health maintenance, ensuring they understand their care plans and how to manage their health effectively. Collaboration is key in this position; you will work closely with physicians, therapists, and other healthcare professionals to coordinate and adjust care plans as needed. Accurate documentation of all patient interactions, treatments, and assessments is essential, in accordance with agency policies and regulatory requirements. You will ensure compliance with all state, federal, and agency regulations and policies, including infection control and safety standards. Additionally, you will act as a liaison between patients, families, and healthcare providers to address patient needs and concerns, providing support and training to promote independence and health improvement.
