Requirements
- Current Florida Registered Nurse (RN) license (required)
- Excellent assessment, communication, time management, and documentation skills

Nice To Haves
- Previous home health experience is strongly preferred but not required
- OASIS experience preferred
Responsibilities
- Conduct OASIS assessments and complete state-of-care, recertification, resumption-of-care, and discharge visits
- Develop and implement individualized plans of care in collaboration with physicians and the interdisciplinary team
- Perform comprehensive nursing assessments and ongoing patient evaluations
- Administer medications, perform skilled nursing procedures, and provide wound care as ordered
- Educate patients and caregivers on disease management, medications, safety precautions, and infection prevention
- Supervise and coordinate care provided by LPNs, HHAs, and other team members as appropriate
- Monitor patient progress, update care plans, and maintain thorough, timely, and compliant documentation
- Ensure compliance with all Medicare, CMS, and home health regulations
