Requirements
- Active RN license
- 1+ year of nursing experience
- Reliable transportation and valid driver’s license
- Willingness to travel within Lafayette area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
Responsibilities
- Complete Start of Care and follow-up visits
- Assess patients and update care plans
- Educate patients and families
- Coordinate with physicians and care team
- Document visits accurately
