Requirements
- Active Oklahoma RN License
- Current BLS/CPR Certification
- Valid Driver’s License and Reliable Transportation
- Strong Clinical Assessment and Communication Skills
Nice To Haves
- Home Health Experience Preferred
- OASIS Documentation Experience Preferred
Responsibilities
- Complete skilled nursing visits in patients’ homes
- Perform assessments and develop individualized care plans
- Coordinate care with physicians, therapists, and interdisciplinary team
- Educate patients and families on disease management and treatment plans
- Complete timely and accurate documentation
- Monitor patient progress and adjust care as needed
