Concierge Home Care is hiring a Weekend Home Health Registered Nurse (RN) in the Orange Park area. This modified full-time role is designed for nurses who want more flexibility, fewer workdays, and higher earning potential—without sacrificing patient care quality.

(Territories designed to minimize drive time with clustered scheduling)

Requirements

• Active, unencumbered Florida Registered Nurse (RN) license
• Valid driver’s license, reliable transportation, and auto insurance
• Home health experience preferred, but not required

Responsibilities

• Perform comprehensive patient assessments in the home setting
• Develop and manage individualized care plans
• Provide skilled nursing care and patient/caregiver education
• Administer treatments and monitor patient condition
• Coordinate care with physicians and interdisciplinary teams
• Evaluate patient progress and adjust care as needed
• Complete accurate, timely documentation using streamlined tools
