We travel to patients' homes throughout the day to help patients empower themselves to improve self-care. Lorian Health is currently looking for a Per Diem Registered Nurse (RN) to provide optimum care to our homebound patients in San Diego County. The RN provides nursing care to patients/clients as requested by the patient/client and, when required, as prescribed by the physician and in compliance with applicable laws and regulations and the policies of Lorian Health. As a Home Health Registered Nurse (RN), you will provide care to our homebound patients in San Diego. The RN provides nursing care to patients/clients as prescribed by the physician and in compliance with applicable laws and regulations and the policies of Lorian Health. You will be responsible for hands-on supervision for activities including medical necessity review, initial assessments, continued stay review, discharge planning, and other duties as required. You will be expected to utilize all elements of the nursing process, assess, and evaluate patient/client status, and provide professional nursing care. We have a highly skilled and collaborative team of healthcare professionals including nurses, therapists, and care managers, who work together to provide the best possible care to our patients. Our innovative team nursing model pairs an RN with an LVN and therapists, and CHHA’s. We travel to patients' homes throughout the day. As a Registered Nurse (RN), you will provide professional nursing care by utilizing all elements of the nursing process. You will assess and evaluate patient/client status, administer medications and treatments as prescribed by the physician, and provide and maintain a safe environment for the patient/client. You will also teach the patient/client and family/caregiver self-care techniques as appropriate. Requirements
- graduate of an approved school of professional nursing, with a BSN preferred.
- currently licensed to practice as an RN in the state of practice
- clinical knowledge of various disease processes.
- one year of acute nursing experience in medical, surgical, and/or critical care.
- excellent observation, good nursing judgment, and good oral and written communication skills.
- self-directing, flexible, and cooperative in fulfilling their role obligations.
Nice To Haves
- Home health experience is preferred.

- This position is a Pay per visit compensation model.
- Pay per visit ranges from $50- $160/ commensurate with experience and depending on type of visit. The compensation reflected on this posting is an estimate of yearly compensation.
