Requirements
- Current RN license in the state of Texas
- Minimum 1 year of RN experience (Home Health preferred)
- Proficiency in English (spoken and written)
- Valid Texas driver’s license and current auto liability insurance
- Current CPR/BLS certification
Nice To Haves
- Familiarity with Kinnser/WellSky is a plus
Responsibilities
- Deliver high-quality, individualized nursing care to patients in their homes as prescribed by the physician.
- Collaborate with an interdisciplinary care team to develop, implement, and adjust the patient’s plan of care.
- Monitor and evaluate patient progress, reporting findings to the physician, case manager, and family as appropriate.
- Administer treatments and manage nursing interventions based on clinical assessments.
- Participate in on-call rotation (approximately every 3–4 weeks) and perform PRN visits as needed.
- Complete documentation using Kinnser (WellSky) system.
- Other duties as assigned.
