Requirements

• Current PA Nursing License
• Demonstrated experience in home healthcare or a related field (preferred)

Responsibilities

• Conduct patient assessments and develop individualized care plans for patients receiving home health services.
• Utilize physiology knowledge and nursing expertise to deliver comprehensive care to patients.
• Collaborate with interdisciplinary healthcare team members to ensure continuity of care and positive patient outcomes.
• Demonstrate proficiency in managing care for patients with diverse medical needs, including critical care, ICU, and behavioral health patients.
