Home Health Registered Nurse, RN Visits
About The Position
Valley Health at Home by BAYADA is seeking a compassionate and dedicated Registered Nurse, RN to join our team and work with adult and senior clients on a per visit basis. This is a full time position. At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings) is required. Medicare Home Health and OASIS documentation highly preferred. As a home care Registered Nurse, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Requirements
- A current license as a Registered Nurse in Nevada.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
Nice To Haves
- Prior home care experience strongly preferred, but not required.
- Medicare Home Health and OASIS documentation highly preferred.
Responsibilities
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
