South Broad Street covering North, South, and West Philadelphia 2. Newtown Square covering Chester and Delaware Counties 3. Requirements

BSN required.
Current PA RN License and CPR Certification.
Hospice/Palliative Care Certification required.
Home care experience, strong clinical knowledge base, and hospice experience preferred.
Strong interpersonal, organizational, problem-solving, critical thinking, and priority-setting skills.
Ability to work independently and as part of a team.
Proficient in computer skills.
Graduate of an approved nursing education program.
Licensure as a Registered Nurse in the state of PA.
Palliative Care Certification is required
One (1) year of experience as a professional care nurse in home care or hospice care is a plus.
Must have a current driver’s license and reliable transportation.

Responsibilities

Provide comprehensive care to patients and families in the palliative care program.
Offer clinical consultation in palliative care.
Develop, coordinate, and evaluate palliative and end-of-life care programs.
