Physical Therapy Assistant provides skilled Physical Therapy services on an intermittent basis to clients in their homes, under the direction of the Registered Physical Therapist (PT). How will you make an impact & Requirements Provides client services that have been delegated by- and are under the direction of- the Registered Physical Therapist (PT) Assists the PT in evaluation and development of a rehabilitative Plan of Care and in periodic re-evaluation as required. Participates in the preparation of clinical and daily progress notes which are reviewed by the PT and enters in EMR within 24 hours. Performs routine strengthening and therapeutic exercises and assists the patient with the use of self-help devices, as directed. Assists the PT in providing instructions and in demonstrating exercise programs to the patient and family. Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports same to the PT. Observes and records activities and findings in the clinical record; informs the PT and the Physician of any reactions to treatment and any changes in the client’s condition. Consults with PT regarding the outcome of home visits and reports physical findings in writing. Instructs other health team personnel including the home health aides as well as family members, as appropriate, with respect to certain phases of physical therapy, so that they may work with the client. Writes daily progress notes concerning patients and submits same within 72 hours; prepares periodic summary reports as indicated. Attends Rehabilitation Department staff meetings and case conferences. Participates with staff, clients, and physicians in discharge activities and assists the PT in completing the Physical Therapy Discharge Summary. Confirms, on a weekly basis, the scheduling of visits with the Supervisor/Director of Nursing in order to coordinate Agency visits. Assumes responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organizations and individual. Assumes other duties deemed appropriate and necessary by the PT and as designated by Registered Nurse Case Management and the Nursing Supervisor. Is knowledgeable of the Agency’s organization, functions, and services. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests. Maintains current TB testing (refer to Health Screening Policy).
