About The Position
At AHN Healthcare@Home, we are looking to expand our care team with a Home Health Physical Therapy Assistant. This role involves collaborating to manage and deliver patient care plans in a team-based environment. AHN Healthcare@Home, in partnership with Allegheny Health Network, is an accredited leader in home health and hospice, dedicated to providing high-quality patient care that significantly impacts lives daily. This specific position supports patients in Connellsville, Uniontown, and surrounding areas and includes a $5,000 first-year success bonus for achieving key milestones.
Requirements
- Associate degree in physical therapy from an accredited college or technical school required
- Must hold, in good standing, a license/registration as a Physical Therapist Assistant, issued by the State in which you work
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- A commitment to consistently meet critical deadlines for charting
- The skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
Nice To Haves
- Minimum one year of experience in an acute care and/or rehabilitative setting preferred
- Previous home health experience preferred
