Job Description: Registered Nurse (RN) — Home Health | PRN (Per Diem) At Elara Caring, we believe the best care happens at home. Requirements

  • Current, unrestricted Registered Nurse (RN) license in the state
  • Minimum 1 year of clinical nursing experience (home health strongly preferred)
  • Strong assessment and patient education skills
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to travel within the assigned coverage area
  • Comfortable working independently in a field-based setting
  • Experience with OASIS and electronic documentation (HomeCare HomeBase a plus)

Responsibilities

  • Perform start-of-care, resumption, recertification, and routine skilled nursing visits
  • Conduct comprehensive patient assessments (physical, psychosocial, and environmental)
  • Develop and manage individualized plans of care in collaboration with physicians and the interdisciplinary team
  • Provide patient and caregiver education on medications, disease management, and safety
  • Monitor patient progress and communicate condition changes to the physician and clinical leadership
  • Perform skilled nursing interventions (wound care, medication management, injections, post-hospital follow-up, etc.)
  • Complete timely and accurate documentation (OASIS and visit notes)
  • Coordinate services with therapists, aides, and other care team members
  • Maintain patient privacy and confidentiality in compliance with HIPAA
