Requirements

Education: Associate’s degree from an accredited Physical Therapist Assistant program.
Licensure: Must be licensed to practice as a Physical Therapy Assistant in the State of Indiana.
Experience: At least two years of experience required
Valid driver’s license and automobile insurance
Successful completion of a criminal background check, post-offer physical exam, and drug screening
Flexibility to travel as needed

Nice To Haves

geriatric or home health experience preferred.

Responsibilities

Provide in-home physical therapy treatments under the direction of a licensed Physical Therapist and established plan of care.
Carry out therapeutic exercises and functional activities while monitoring and reporting patient response and progress.
Maintain accurate, timely documentation including visit notes, time, and mileage per agency guidelines.
Communicate and collaborate with the Physical Therapist and interdisciplinary team to ensure coordinated patient care.
