Requirements

Licensed Physical Therapist Assistant (PTA) in the state of Indiana
Graduate of an accredited PTA program (APTA/CAPTE)
Minimum 6 months PTA experience (home health preferred, not required)
Strong communication and documentation skills
Reliable transportation, valid driver’s license, and required auto insurance
Compassionate, dependable, and patient-focused approach

Responsibilities

Provide in-home physical therapy treatments under the supervising PT's plan of care
Teach patients and caregivers proper exercises, mobility techniques, and safety strategies
Monitor and document patient progress accurately and timely
Communicate updates to the supervising Physical Therapist and interdisciplinary team
Deliver care in accordance with state, federal, and professional standards
Promote patient safety and maintain compliance with agency policies
Participate in interdisciplinary collaboration as needed
