Requirements

  • A genuine passion for helping others live healthier, more independent lives
  • Graduate of an accredited Physical Therapy program
  • Current Nebraska Physical Therapy license in good standing
  • Current or eligible NPTE certification
  • At least one year of experience in an acute care, rehab, or outpatient setting
  • Strong clinical judgment and the ability to work independently
  • Reliable transportation to visit patients in their homes

Nice To Haves

  • Home health experience (2+ years preferred)

Responsibilities

  • Help patients restore function, improve mobility, reduce pain, and prevent disability
  • Support patients in achieving the highest possible level of physical independence
  • Participate in interdisciplinary care planning with the full home care team
  • Perform comprehensive initial evaluations, OASIS Start of Cares (SOCs), and develop individualized plans of care
  • Administer prescribed physical therapy treatments and skilled interventions
  • Conduct 30‑day reassessments, adjust care plans, and update documentation as needed
  • Provide instruction to the patient and family on safe mobility, strengthening, and home programs
  • Supervise up to two Physical Therapist Assistants (PTAs) in accordance with Nebraska state guidelines and agency policies
