About The Position
Sentara Home Care is hiring a Full-Time Physical Therapist in Chesapeake, Virginia This is a Full-Time, Day shift opportunity with great benefits. The schedule is 8 am - 5 pm on Mondays - Fridays and a holiday rotation As a Physical Therapist (PT) with Sentara Home Care, you assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury. Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members. Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures. Provides effective instruction to the patient and family through standardized educational programs and/or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes. Productive and efficient in daily operations. Selects and performs the appropriate assessment procedures. Establishes and updates formal plan of care to address individual patient needs. Required license must be valid in state of practice.
Requirements
- Bachelor's Level Degree
- Physical Therapy license in state of practice
- At least 2 years of experience is required
