Home Health Physical Therapist (PT)
About The Position
As a Home Health Physical Therapist, you will provide skilled, one-on-one therapy services in the comfort of patients’ homes, helping them regain strength, mobility, and independence. This role involves assessing patient needs, developing individualized treatment plans, delivering evidence-based interventions, and educating patients and caregivers to support safe recovery at home. The position offers the opportunity to practice at the top of your license with greater flexibility, autonomy, and meaningful 1:1 patient relationships, whether you are experienced in home health or transitioning from other clinical settings. Holy Cross Home Care and Hospice, a member of Trinity Health At Home, is a faith-based, not-for-profit agency committed to caring for patients and families at home, offering services including skilled nursing, physical, occupational, and speech therapy, and medical social work.
Requirements
- Current licensure as a Physical Therapist in the State of Maryland
- Graduate of an accredited physical therapy program
- At least one year of clinical experience
- Valid driver’s license and reliable transportation
- Ability to work independently and manage time effectively

- Pay Range $38.81-$62.09 per hour (based on years of experience, skillset, education, etc.)
