About The Position

A Home Health Physical Therapist delivers skilled rehabilitation services in patients’ homes to improve mobility, strength, balance, and overall functional independence. Care is individualized and focused on helping patients safely recover, manage chronic conditions, and maintain independence in their daily environment. The objective of this role is to enhance patients’ mobility, independence, and quality of life by delivering effective physical therapy services in the home setting.

Requirements

• Doctor of Physical Therapy (DPT) or equivalent degree
• Active Physical Therapist license in the state of practice
• CPR certification (or willingness to obtain)
• Valid driver’s license and reliable transportation

Nice To Haves

• Home health or outpatient experience preferred

Responsibilities

• Conduct in-home evaluations of patients’ physical function, mobility, and safety needs
• Develop and implement individualized treatment plans based on clinical findings and patient goals
• Provide hands-on therapy including therapeutic exercise, gait training, balance training, and functional mobility training
• Evaluate home environments for safety risks and recommend adaptive equipment or modifications
• Educate patients and caregivers on safe mobility techniques, fall prevention, and home exercise programs
• Monitor and document patient progress, adjusting treatment plans as needed
• Maintain accurate and timely clinical documentation in compliance with agency standards
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
• Travel to patient homes within assigned service area
