A great physical therapist works with more than just the patient, involving the physician, family and other healthcare providers as needed. A great physical therapist doesn’t just go through the motions, instead listening carefully to a patient’s concerns, monitoring progress, and adjusting plans as needed. We are looking for a great physical therapist. WHAT YOU CAN EXPECT Completes appropriate goal re-assessment and discharge planning. Performs comprehensive physical therapy evaluations, treatment, and related follow-up services including discharge planning. Develops a treatment plan with measurable goals based on patient assessment. Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care. Documents patient assessments and pertinent information in a timely manner. Provides services and treatments requiring physical therapy skills according to the written plan of care and the signed plan of treatment. Supervises Physical Therapist Assistants and Home Health Aides. Provides physical therapy according to standards of care defined by the state practice act and other professional and governing agencies. Trains and educates patients and family members or others support and documents understanding of treatment plan and goals. Prime Healthcare will add the 214‑bed Franciscan Health Olympia Fields Hospital and Franciscan’s Illinois operations, including Specialty Physicians of Illinois LLC, Working Well’s Illinois operations, Franciscan Health & Fitness Center Chicago Heights, Home Care, and Chicago Heights Walk-In Clinic, to its system. This acquisition remains subject to customary regulatory review and approvals. Requirements

  • Bachelor's Degree- Physical Therapy - Required
  • 1 Year Physical Medicine/Rehabilitation Experience- Required
  • Physical Therapist (PT) State Licensing Board - Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
  • Driver's License - Required
  • Auto Insurance - Required

Nice To Haves

  • 1 Year Home Health Experience - Preferred
