About The Position
The Home Health Physical Therapist (Part Time/PRN) plays a critical role in delivering personalized rehabilitative care to patients within their homes across the United States. This position focuses on improving patients' mobility, reducing pain, and enhancing overall physical function through tailored therapeutic interventions. The therapist will assess patient needs, develop individualized treatment plans, and monitor progress to ensure optimal recovery outcomes. Collaboration with interdisciplinary healthcare teams and effective communication with patients and their families are essential to provide holistic care. Ultimately, this role aims to empower patients to regain independence and improve their quality of life in a comfortable, home-based setting.
Requirements
- Successful completion of accredited physical therapy program and state licensure exam.
- Valid Physical Therapy license
- CPR certification
- Valid Driver’s license
- Valid Auto Insurance
Nice To Haves
- 1 year of home health experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
- Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Applies concepts of infection control/universal precautions in performing patient care
- Assure continuity of quality patient care delivered with appropriate documentation
- Provide effective communication to patient/family, team members and other health care professionals
- Promote agency philosophy and administrative policies
- Maintain and complete all documentation within company stated guidelines.
