Requirements

• Graduate of an APTA-accredited Physical Therapy program
• Current, unrestricted PT license in the state of Michigan
• At least 1 year of experience in a healthcare setting
• Reliable transportation , valid driver’s license, and current auto insurance
• Ability to travel up to 50% within your coverage area
• Physical ability to lift, transfer, and assist patients (50–100 lbs)

Responsibilities

• Deliver direct patient care and evaluate functional needs to design effective treatment plans.
• Develop and implement personalized care plans based on thorough assessments and patient goals.
• Collaborate with physicians, nurses, and other clinicians to ensure optimal outcomes.
• Guide and direct support staff to help patients meet their functional goals.
• Maintain accurate, timely documentation of evaluations, goals, and progress notes.
• Promote Elara Caring’s mission and represent physical therapy services within the community.
