Requirements

Active Physical Therapist license in the State of Pennsylvania
At least 1 year of recent experience
Valid Pennsylvania Driver’s License with no restrictions and a car with required insurance
Satisfactory Criminal Record Clearance

Responsibilities

Provide comprehensive patient care through assessment and treatment planning in home health settings.
Utilize knowledge of physiology and anatomy to develop effective physical therapy interventions.
Collaborate with healthcare teams to create and implement care plans, ensuring continuity of care.
Educate patients and families on therapeutic exercises and discharge planning.
Maintain accurate medical records and documentation in accordance with regulations.
