About The Position

Home Health Physical Therapist (PT) | Robertson, Cheatham and Sumner County Area | Full Time All Heart Home Care is a mission driven health care organization committed to compassionate, high quality care. Our team is dedicated to supporting patients with respect, empathy, and individualized care that honors their independence and overall well being. We are seeking a Physical Therapist to deliver home health services to patients recovering from injury, illness, or medical procedures. This role focuses on helping individuals improve mobility, manage pain, rebuild strength, and safely return to daily activities. Providing care in the home allows for personalized treatment plans that meet patients where they are most comfortable, while reducing barriers to recovery and lowering the risk of hospitalization. The Physical Therapist will work with the support of an experienced home health leadership team and a collaborative interdisciplinary staff.

Requirements

• Active, valid, and unencumbered Tennessee Physical Therapist license
• Current driver license with reliable transportation and active auto insurance
• One year or more of clinical experience preferred
• Ability to work independently while maintaining strong organizational skills
• Commitment to patient focused care, professionalism, and compassionate service
• Must be able to pass background and drug screening

Responsibilities

• Delivering therapeutic interventions
• Providing education to patients and caregivers
