Looking for a Part-Time Home Health Physical Therapist provide care for our home care Prince George County, MD. Position Overview - $6,000 Sign-On Bonus As a Home Care Physical Therapist (PT) at Holy Cross Home, you'll provide skilled physical therapy services to patients in the comfort of their own homes, enabling recovery, improving mobility, and enhancing quality of life. Requirements
- Current licensure as a Physical Therapist in the State of Maryland
- Graduate of an accredited physical therapy program.
- At least one year of clinical experience (home care experience preferred).
- Valid driver's license and reliable transportation.
- Ability to work independently and manage time effectively.

Responsibilities
- Assess patients' functional status and physical therapy needs in their homes.
- Develop and implement individualized care plans.
- Provide direct physical therapy services in accordance with the physician's plan of care.
- Educate patients, families, and caregivers on exercises and therapy techniques.

- Pay Range $38.81-$62.09 per hour (based on years of experience, skillset, education, etc.)
