Requirements

  • Master's degree in Physical Therapy from an accredited program.
  • Valid state licensure as a Physical Therapist.
  • Previous experience in home health care or outpatient clinical setting preferred.
  • Strong interpersonal and communication skills to build rapport with clients and their families.
  • Ability to work independently while managing a varied caseload effectively.
  • Familiarity with electronic health records and documentation standards.
  • Commitment to upholding the company's values of compassion, honesty, and patience.

  • Previous experience in home health care or outpatient clinical setting preferred.

Responsibilities

  • Provide comprehensive physical therapy evaluations and ongoing assessments for patients in their home environments.
  • Develop and implement individualized treatment plans based on evaluations and clinical goals.
  • Educate patients and their families about therapeutic exercises, activities, and the maintenance of their physical health.
  • Monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes.
  • Work collaboratively with other healthcare professionals to coordinate care and improve patient outcomes.
  • Document patient care and progress according to regulatory standards and company policies.
  • Stay current with best practices and advancements in physical therapy to incorporate into treatment.
