Requirements
- Active Physical Therapist license in the state of Texas (TX)
- Graduate degree or higher in Physical Therapy from an accredited program
- Previous experience in home health or rehabilitative settings is preferred
- Ability to adapt care approaches to meet the individual needs of patients
- Up-to-date knowledge of evidence-based treatment techniques and best practices
- Excellent communication skills and the ability to maintain a professional, patient-centered demeanor when interacting with patients and families

- Kinnser/Wellsky EMR experience preferred
Responsibilities
- Evaluate Patients: Assess patients’ physical abilities and functional limitations in a home health setting to develop comprehensive care plans.
- Develop and Implement Treatment Plans: Create individualized treatment plans aimed at restoring function, improving mobility, and managing pain.
- Patient and Caregiver Education: Educate patients and their families on exercise programs, safety strategies, and adaptive techniques to support ongoing care.
- Resumption Of Care Visits: Monitor and document patient progress, making necessary adjustments to treatment plans to ensure optimal outcomes.
- Deliver Compassionate Care: Provide high-quality, empathetic care, with a focus on empowering patients to improve their independence and quality of life.
