Requirements

• Degree from an APTA-accredited physical therapy program
• Active state license to practice as a Physical Therapist
• At least 1 year of experience or completion of a 6–8 week preceptorship
• Strong organizational, communication, and tech skills
• Valid driver’s license and reliable transportation

Nice To Haves

• Home health experience and knowledge of Medicare Conditions of Participation preferred

Responsibilities

• Deliver high-quality physical therapy services in a home setting
• Conduct assessments and develop individualized care plans
• Educate patients and caregivers on exercises and mobility techniques
• Collaborate with interdisciplinary teams to ensure holistic care
• Use advanced, user-friendly technology to document and track progress
