We are seeking a Physical Therapist to deliver home health services to patients recovering from injury, illness, or medical procedures. This role focuses on helping individuals improve mobility, manage pain, rebuild strength, and safely return to daily activities. Providing care in the home allows for personalized treatment plans that meet patients where they are most comfortable, while reducing barriers to recovery and lowering the risk of hospitalization. The Physical Therapist will work with the support of an experienced home health leadership team and a collaborative interdisciplinary staff. Responsibilities include conducting initial and ongoing assessments, designing and updating individualized plans of care, delivering therapeutic interventions, and providing education to patients and caregivers. Clear communication with physicians and care team members is essential to ensure continuity of care and positive outcomes.
