Requirements

Education: Bachelor’s degree in Physical Therapy from an accredited school.
Licensure: Must be licensed to practice as a Physical Therapist in the State of Indiana.
Experience: At least two years of experience required; geriatric or home health experience preferred.
Valid driver’s license and automobile insurance
Successful completion of a criminal background check, post-offer physical exam, and drug screening
Flexibility to travel as needed

Responsibilities

Evaluate patients referred by a physician, focusing on diagnosis-specific assessments.
Maintain accurate and timely documentation, including daily time and mileage records, evaluations, and progress notes, in accordance with agency policies.
Schedule patients for treatment as needed.
Collaborate with other healthcare providers and disciplines to ensure cohesive patient care.
Communicate with the physician and VNA staff regarding patient’s treatment progress and status.
