Home Health Physical Therapist (PT)
About The Position
Windsor Home Health is seeking a Physical Therapist for home health services on a Part-time and/or PRN basis. Responsibilities
- Interdisciplinary discussion/review of patients on schedule and treatment
- New admissions evaluations
- Review of potential discharges
- Staff training and/or communication of patient progress
- Programming: Wheelchair, splints, adaptive equipment
- Communication with family of patients with progress/updated goals/adaptive equipment/change of status
