We are seeking a Part-Time/PRN Physical Therapist (PT) to join our home health team in Chicago, specifically Downtown, Lakeview, Edgebrook, Streeterville, Norridge, Harwood Heights, and Humboldt Park. Requirements
- Licensed Physical Therapist (PT)
- Doctorate in Physical Therapy (DPT)
- Strong communication and documentation skills
- ability to work independently in a home-based setting
- professional, dependable, and patient-focused approach
- Physical ability to perform in-home visits
Nice To Haves
- Home Health experience preferred, but not required
Responsibilities
- Perform in-home physical therapy evaluations and treatments
- Develop and implement individualized rehabilitation plans of care
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises, mobility, and safety
- Document visits accurately and efficiently in accordance with agency guidelines
- Communicate patient progress with the interdisciplinary care team
- Ensure compliance with clinical, documentation, and regulatory standards
