Requirements

  • Degree from an APTA-accredited Physical Therapy program
  • Active Physical Therapist license in the state of Michigan
  • Minimum of 1 year of clinical experience (home health preferred, not required)
  • Comfort working independently in the field with strong clinical judgment
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to travel within the service area (up to 50%)
  • Physical ability to assist, lift, and transfer patients (up to 50–100 lbs)

Responsibilities

  • Conduct in-home evaluations to assess mobility, strength, balance, and functional limitations
  • Create and adjust personalized therapy programs aligned with physician orders and patient goals
  • Provide hands-on treatment focused on restoring independence and preventing re-injury
  • Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated care
  • Educate patients and caregivers on safe mobility, exercises, and fall prevention
  • Supervise and support therapy assistants as needed
  • Complete accurate and timely documentation in compliance with agency and regulatory standards
  • Serve as a trusted therapy resource within the communities you support
