Requirements
- Active Nevada Physical Therapist license
- Home health experience required
- Current CPR certification
- Strong clinical judgment and communication skills
- Reliable vehicle and valid Nevada driver’s license
- Commitment to patient-centered, high-quality care
Responsibilities
- Perform in-home physical therapy evaluations
- Develop and implement individualized treatment plans
- Improve patient mobility, safety, and functional independence
- Educate patients, families, and caregivers to support recovery
- Collaborate with interdisciplinary care teams
- Complete accurate documentation using AI-assisted tools
