Requirements
- Current Florida Physical Therapist (PT) license (required)
- Excellent communication, time management, and documentation skills
- A compassionate, dependable, and professional attitude
- A team-oriented mindset and a genuine passion for making a difference in patients’ lives
Nice To Haves
- Previous home health experience is strongly preferred but not required
Responsibilities
- Conduct OASIS assessments and initial start-of-care visits to establish individualized plans of care
- Evaluate functional limitations and develop personalized therapy goals and interventions
- Guide patients through therapeutic exercises and mobility programs to maximize independence
- Educate patients and caregivers on safe home exercise programs and fall-prevention techniques
- Collaborate closely with physicians, nurses, and other care-team members to coordinate care
- Monitor patient progress, update care plans, and maintain thorough, compliant documentation
- Ensure compliance with all Medicare, CMS, and home health standards
