We are looking for a Physical Therapist to join our team with towns including Downtown Chicago, Lakeview, Edgebrook, Streeterville, Norridge, Harwood Heights, Humboldt Park, Skokie, Evanston, Rogers Park, and Lincolnwood. Requirements

Licensed Physical Therapist (PT)
Doctorate in Physical Therapy (DPT)
Strong communication and documentation skills
ability to work independently in a home-based setting
professional, dependable, and patient-focused approach
Physical ability to perform in-home visits

Nice To Haves

Home Health experience preferred, but not required

Responsibilities

Perform in-home physical therapy evaluations and treatments
Develop and implement individualized rehabilitation plans of care
Monitor patient progress and adjust treatment plans as needed
Educate patients and caregivers on exercises, mobility, and safety
Document visits accurately and efficiently in accordance with agency guidelines
Communicate patient progress with the interdisciplinary care team
Ensure compliance with clinical, documentation, and regulatory standards
Supervise PTA field staff as indicated
