About The Position

Classic Home Care, a nurse-owned and operated skilled home health agency, has been proudly serving communities across Southeast Michigan since 2003. While our office is located in Northville, Michigan, we provide exceptional care to patients in many surrounding areas. We are Joint Commission-accredited and Medicare-certified, ensuring the highest standards of quality and patient care. Our agency is led by registered nurses (RNs) who actively collaborate with our dedicated team, fostering a supportive and professional environment. We are seeking dedicated and compassionate Physical Therapists (PTs) to join our team as home health / home care PTs! Contingent PT: A great opportunity for those looking to start with flexible hours and transition into a full-time or part-time caseload over time. Requirements

  • Active Michigan Physical Therapy license
  • Reliable transportation
  • Valid driver's license and auto insurance in your name as a driver

Nice To Haves

  • Home health care experience preferred

Responsibilities

  • Provides skilled physical therapy in accordance with the physician’s plan of care. Accepts only patients referred by the patient care manager or designee.
  • Assists the physician in assessing the patient’s functional level by applying appropriate tests and assists in the development and revision of the plan of care, including OASIS assessments and goal setting as appropriate.
  • Treats patients to relieve pain, develop and restore function and maintain maximum performance, using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care. Identifies barriers to desired outcomes.
  • Observes, records and reports the patient’s reaction to treatment and any change in the patient’s condition to the physician and/or patient care manager and other staff involved.
  • Instructs and advises patient, family and other health team personnel including, when appropriate, the physical therapy assistant and home health aides, in the phases of physical therapy in which they may assist the patient; instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate. The goal is to teach and promote disease prevention and injury management for eventual patient & caregiver independence, utilizing agency materials as appropriate.
