About The Position

We’re looking for a part-time Physical Therapist Assistant to join our home health team and support patients across South Lyon, Plymouth, and Walled Lake. You’ll partner closely with a supervising Physical Therapist to help patients build strength, increase mobility, and improve their overall quality of life. No holidays. No weekends. Just purposeful work and a supportive team behind you.

Requirements

Graduate of an accredited PTA program recognized by the American Physical Therapy Association.
Active, unrestricted PTA license for the state of Michigan.
At least six months of PTA experience (home health experience a plus).
Reliable transportation with valid driver’s license and current auto insurance.
Ability to travel within the service area (up to 50%).
Physical capacity to lift 50–100 lbs and perform frequent movement throughout the day.

Responsibilities

Provide hands-on therapy interventions according to the established Plan of Care.
Support patients in achieving greater independence through mobility training, strengthening exercises, and safe use of assistive equipment.
Monitor patient progress and alert the supervising PT to changes in condition or care needs.
Deliver care in alignment with state practice regulations and Elara Caring standards.
Teach patients and caregivers exercises, safety strategies, and principles for success at home.
Engage in physically active work—lifting, transferring, and assisting patients as appropriate.
