Requirements
- Current Florida Physical Therapist Assistant (PTA) license (required)
- Excellent communication, time management, and documentation skills

- A team-oriented mindset and a genuine passion for making a difference in patients’ lives
Nice To Haves
- Previous home health experience is strongly preferred but not required
Responsibilities
- Provide skilled physical therapy treatments in patients’ homes under the direction of a licensed Physical Therapist
- Carry out individualized treatment plans focused on mobility, strength, balance, and functional independence
- Assist patients with therapeutic exercises, gait training, transfers, and mobility training
- Educate patients and caregivers on safe mobility techniques, home exercise programs, and fall prevention strategies
- Observe, document, and report patient responses and progress to the supervising Physical Therapist
- Communicate effectively with physicians, nurses, and other members of the interdisciplinary care team
- Maintain accurate, timely, and compliant clinical documentation
- Ensure compliance with all Medicare, CMS, and home health regulations
