About The Position

The Home Health Physical Therapist at Kaiser Permanente is responsible for providing independent, age-appropriate physical therapy to patients in their homes. This role involves evaluating patients, developing treatment plans, and coordinating care with other healthcare professionals. The therapist will also educate patients and their families on preventative and rehabilitative procedures, ensuring effective treatment and discharge planning.

Requirements

  • Minimum two (2) years of experience in physical therapy.
  • Graduate of an accredited physical therapy curriculum with a bachelor's, master's, or doctoral degree.
  • California Driver's License required at hire.
  • California Physical Therapist License required at hire.
  • National Provider Identifier required at hire.
  • Basic Life Support certification.

Nice To Haves

  • Experience in an automated clinical information system preferred.
  • Computer literate preferred.
  • Experience in assessment of high mobility equipment preferred.

Responsibilities

  • Evaluates patients and interprets evaluation findings to determine the nature and extent of dysfunction.
  • Establishes treatment goals and plans treatment to achieve established goals.
  • Initiates discharge planning for physical therapy.
  • Implements the plan of care through direct treatment of the patient and coordinates the treatment program with other healthcare personnel.
  • Provides advice and education to patient and family.
  • Regularly reassesses clinical signs and symptoms to determine effectiveness of treatment and progress towards goals.
  • Assesses for adaptive equipment for Activities of Daily Living based on clinical guidelines.
  • Coordinates adaptive equipment assessment with certified equipment vendors.
  • Discharges patient from physical therapy when maximum benefit has been received.
  • Participates in educational and clinical research activities in physical therapy.
  • Assists in the development, implementation, and review of departmental patient care programs.
  • Supervises the work of Home Health Aides according to regulation and agency policy.
  • Provides accurate, timely documentation of physical therapy patient management.
