Requirements

• Current RN licensure in state of practice.
• Current CPR certification required.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Supervise and coordinate clinical services and provide nursing care to patients in their homes as prescribed by the physician.
• Ensure compliance with applicable laws, regulations, and agency policies.
• Provide clinical services within the scope of practice as defined by state laws governing nursing.
• Receive referrals and ensure appropriate clinician assignments.
• Evaluate patient orders and plot start of care visits.
• Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Assess patient needs continually and ensure care rendered is individualized, appropriate, and meets home health eligibility criteria.
• Review assessments and plans of care daily and consult clinicians with recommendations as appropriate.
