About The Position

The Occupational Therapist position at Advocate Health involves providing quality, cost-effective care to patients in their homes or assisted living facilities. Requirements

  • Master's Degree in Occupational Therapy.
  • Licensed in the State of Illinois (or eligible for licensure).
  • 1 year of clinical experience or strong clinical affiliation experience.
  • Graduated from an Occupational Therapy curriculum accredited by the American Occupational Therapy Association.
  • Basic knowledge of computer use (i.e. Microsoft Word, email).
  • Ability to communicate professionally both verbally and in writing.
  • Good interpersonal skills and time management abilities.
  • Proficient use of home-based therapy equipment and assistive/adaptive devices.

Nice To Haves

  • Experience in home health care settings.
  • Familiarity with electronic health record systems.

Responsibilities

  • Facilitates the patient and family's right to receive quality cost-effective care.
  • Utilizes appropriate resources to respond to situations that may negatively impact outcomes.
  • Adapts practice to the latest standards according to evidence-based literature.
  • Participates in quality/process improvement initiatives.
  • Promotes efforts to reduce risk and improve patient safety.
  • Documents thoroughly to support reimbursement and regulatory requirements.
  • Practices as an effective member of the patient care team.
  • Makes sound clinical decisions for all patients, including those with complex problems.
  • Evaluates and implements changes to the plan of care.
  • Initiates referrals and discharge planning for assigned patients.
  • Participates in interdisciplinary case conferences.
  • Demonstrates respect and understanding for peers and other clinical disciplines.
  • Participates in staff meetings and educational activities.
  • Maintains current knowledge base relevant to practice.
  • Develops and maintains a restorative relationship with patients.
  • Incorporates individualized approaches to the plan of care.
  • Promotes patient/family knowledge and confidence in self-care.
  • Demonstrates behaviors that enhance productivity and understanding of financial conditions.
  • Monitors utilization of resources to achieve excellent health outcomes.
  • Assesses patient and family readiness to learn and modifies approach as necessary.
