About the Role: We are seeking an Occupational Therapist (OT) to provide in-home, individualized therapy to adult clients in Colorado Springs, CO. You’ll support patients in regaining independence and achieving functional goals through skilled, evidence-based care. Requirements

  • A valid Occupational Therapist license in Colorado
  • Proven experience in home health or a related field
  • Excellent communication and interpersonal skills
  • Proficiency in electronic medical record documentation
  • A flexible and adaptive approach to meet the diverse needs of your clients

Responsibilities

  • Develop and implement individualized treatment plans
  • Conduct evaluations using standardized tools and clinical expertise
  • Provide therapy focused on ADLs, mobility, safety, and independence
  • Educate patients and caregivers on home programs and adaptive equipment
  • Maintain accurate and timely clinical documentation
  • Collaborate with a multidisciplinary care team to ensure coordinated patient care
