The Home Health Occupational Therapist uses an individualized patient care plan to provide top quality care to all of our patients; keeping them Happy, Healthy and at Home. We are seeking a passionate Occupational Therapist for our weekday team in Guilford County. This position is Full-Time Monday- Friday 8am-5pm. Full benefits including: Medical, Dental, Vision, 401k with matching, and immediate PTO accrual. Occupational Therapists visit 5-6 patients on any given shift, driving to the patients home providing skilled therapy services. This position is primarily responsible for the initial assessment and case development of patients in all stages of life in their homes. Duties include but not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis. Occupational Therapists (OTs) with strong assessment and documentation skills are strongly encouraged to apply! If this sounds like you, we'd love to hear from you! Requirements

Responsibilities

Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
Educate patients and families on health management, medications, and self-care techniques.
Collaborate with the healthcare team to ensure the highest quality care for each patient.
Occupational Therapists visit 5-6 patients on any given shift, driving to the patients home providing skilled therapy services.
