About The Position

The Home Health Occupational Therapist (OT) evaluates and implements a plan of care to improve, restore and enhance physical functional performance and promote injury prevention. This role also performs documentation and quality improvement activities as outlined in departmental policies and procedures. WHAT YOU CAN EXPECT Performs comprehensive occupational therapy evaluations, treatment and related follow-up services including discharge planning. Documents patient assessments and pertinent information in a timely manner. Develops a treatment plan with measurable goals based on patient assessment. Completes appropriate goal re-assessment and discharge planning. Provides occupational therapy, including selection and use of adaptive equipment. Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care. Trains and educate patients and family members or others support and document understanding of treatment plan and goals. Provides occupational therapy according to standards of care defined by the state practice act and other professional and governing agencies. Supervises Occupational Therapist Assistants and Home Health Aides.

Requirements

  • Bachelor's Degree- Occupational Therapy - Required
  • 1 year Occupational Therapy Experience - Required
  • Occupational Therapist (OT) State Licensing Board - Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
  • Driver's License - Required
  • Auto Insurance - Required

Nice To Haves

  • 1 year Home Health Experience - Preferred
