Requirements

Bachelor of Science in Occupational Therapy.
Active state license as an occupational therapist or meets the requirements listed in 42 CFR §484.115(f). Military spouses with an equivalent out-of-state license are welcome.
A minimum of one year of home health experience preferred.
Proficient therapy skills according to accepted standards of practice, good interpersonal skills, CPR certification, knowledge of durable medical equipment.
Reliable transportation, valid auto liability insurance, and current driver’s license.

Nice To Haves

Familiarity with HomeCare HomeBase software is a plus.

Responsibilities

Perform ongoing clinical assessments to determine patient needs.
Evaluate patients’ occupational performance in self-care, work, leisure, and rest.
Assess and facilitate therapeutic interventions for ROM, strength, coordination, adaptive equipment, and functional mobility.
Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.
Document clinical notes, phone orders, and other records promptly and accurately.
Provide education and counseling to patients and caregivers.
Supervise and guide occupational therapy assistants and home health aides as necessary.
Apply infection control and standard precautions during patient care activities.
Communicate effectively with physicians and healthcare practitioners involved in the patient’s care.
Meet continuing education requirements and demonstrate professional growth and competency.
Promote the Agency’s philosophy and policies to ensure high-quality care.
