About The Position

Indiana Masonic Home Compass Park is looking to add an Occupational Therapist for Home Health. Compass Park is a vast 320-acre community in the heart of Johnson County adjacent to Indianapolis. It is a vibrant community through all seasons and a place of beauty, rich in tall trees and winding footpaths. This job is for Columbus/Franklin/South Indy area. Position Summary The Home Health Occupational Therapist (OT) provides skilled occupational therapy services to patients in their place of residence on an intermittent basis. This role is carried out in alignment with physician orders, the individualized plan of care, and under the direction of the Director of Clinical Services/Clinical Manager. The OT is instrumental in enhancing patients' independence and quality of life while ensuring safety and optimal function in the home setting.

Requirements

• Graduation from an accredited Occupational Therapy program recognized by the American Occupational Therapy Association (AOTA) and the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Successful completion of the National Certification Examination for Occupational Therapists or eligibility to sit for the exam.
• Current licensure in the state(s) of practice (if applicable).
• Minimum of two (2) years of professional experience preferred.
• Strong clinical and interpersonal skills with the ability to work independently in a home health setting.
• Proficiency in documentation and use of electronic medical records (EMR).

Responsibilities

• Patient Care and Therapy Delivery: Conduct initial evaluations to assess patient needs, capabilities, and home environment.
• Develop and implement individualized treatment plans with specific goals tailored to each patient’s unique needs.
• Perform skilled therapy interventions to improve or minimize physical, cognitive, or emotional disabilities, focusing on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Provide patient and caregiver education on therapeutic exercises, adaptive techniques, and safety practices to optimize recovery and function.
• Collaboration and Communication: Collaborate with physicians and interdisciplinary team members to review, adjust, and implement patient care plans as needed.
• Communicate progress, changes in condition, and treatment updates through clear and timely documentation and verbal reporting.
• Participate in care planning meetings with other home health professionals to ensure comprehensive and coordinated care.
• Documentation and Compliance: Maintain accurate, detailed clinical and progress notes for each patient visit in compliance with agency policies and regulatory standards.
• Submit evaluations, recertification, and discharge summaries to the physician for approval as required.
• Ensure compliance with all state, federal, and agency regulations governing home health services.
• Supervision and Leadership: Direct and supervise support staff (e.g., Certified Occupational Therapy Assistants) as required.
• Provide guidance and training to family members and caregivers to support ongoing patient care.
• Participate in quality assurance and performance improvement (QAPI) initiatives as assigned.
• Professional Development: Stay current on best practices, new techniques, and advancements in occupational therapy through continuing education and professional development.
• Periodically present findings or techniques to agency staff to foster knowledge-sharing and team growth.
